 Assistant MDS Coordinator

Asst. MDS Coordinator

JOB SUMMARY:

Under the direction of the MDS Coordinator, responsible for ensuring accuracy and completeness of assigned MDS assessments so that submission is timely and reflects state and federal regulations to meet CMS survey process and PPS reviews. Assists MDS Coordinator in researching quality measure discrepancies and educating staff on proper coding requirements.  Also assists MDS Coordinator in educating and providing in-service training of the MDS process to staff as needed.

QUALIFICATIONS:

Duties require knowledge of MDS and CMS/OBRA/PPS requirements. Completion of an Associate’s Degree and two to five years of related experience, or equivalent combination of education and experience. Experience regarding interpretation of quality measure data and the development of systems to address and resolve flagging areas on the CASPER report is highly desired.  Must be licensed as a Registered Nurse or Licensed Practical Nurse in the State of New Hampshire.

Substance Abuse Counselor

JOB SUMMARY:

Must be a Certified Recovery Support Worker (or in the process of obtaining certification) within the state of New Hampshire. Responsible for assisting the Programs Supervisor in developing, coordinating and delivering quality substance abuse programs and services to inmates.  Must be able to interpret grant funding requirement, produce statistical data and provide data to agencies that authorize grants for services. Knowledge of substance abuse prevention treatment, rehabilitation and physiological symptoms.  Knowledge of detoxification practices for alcohol and/or other drugs.  Knowledge of mental health issues in corrections and experience with Correctional education of inmates.

JOB SUMMARY:

Under the direction of the Medical Coordinator, delivers care to inmates at the Grafton County Department of Corrections by utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in accordance with established philosophy. Practicing specialty nursing in a complex environment which combines the demands of two systems, corrections and health care. Works with a population where serious medical conditions, psychiatric disorders, substance addiction, and infectious diseases are more prevalent then in the general population.

QUALIFICATIONS:

Nursing equivalent of an Associate’s Degree in Nursing. Must be licensed as a Registered Nurse by the State of New Hampshire and three to five years of experience in related field.  Acute care experience and knowledge of mental issues in Corrections.  Ability to be detail oriented and handle multiple tasks.  Candidates must submit to a thorough background investigation and criminal record check
